SBIRT in Integrated Care
Screening, Brief Intervention, and Referral to Treatment (SBIRT) is an evidence-based, population health approach used within integrated care settings to identify and address substance use and behavioral health risks early. SBIRT is seamlessly embedded into routine medical visits—allowing primary care teams to screen universally, deliver brief, targeted behavioral interventions (often using motivational interviewing), and coordinate referrals for patients needing more intensive treatment. SBIRT strengthens integrated care by improving early detection, reducing stigma, enhancing team-based care, and supporting whole-person health outcomes
